Comprehensive Geriatric Evaluation, Care Plan and Monitoring.

The geriatric evaluation is designed to recognize the uniqueness and individuality of a person's needs. While aiding in the diagnosis of medical conditions, an evaluation primarily looks at an older person's physical and emotional health, as well as spiritual, social and environmental conditions. The geriatric evaluation is conducted in the home and can serve to support and guide other service and health care providers.
